aSource: Social network survey of SSI/SSDI Outreach, Access and Recovery (SOAR) stakeholders, including individuals leading SOAR efforts within the community, SOAR practitioners and their supervisors, SOAR trainers, Social Security Administration (SSA) field office staff, staff at state Disability Determination Services (DDS) offices responsible for determining whether an applicant has a qualifying disability, and health care providers. The number of stakeholders in each site varied. Stakeholders in site 11 were not asked about DDS because it was not an active stakeholder at that site at the time of the survey.
